• Enrollment Agreement with $100 Confirmation Fee & $300 Advance Tuition Deposit
• Medical Forms to be completed by a licensed physician
• Application for On-Campus Residence/Residence Hall Agreement (PDF 81 KB)
• Support Services Eligibility Form (PDF 57 KB)
• Application for the Transfer of Credits (PDF 93 KB), if applicable
